ACKNOWLEDGEMENT OF RISK
I, the undersigned, understand and acknowledge that participation in martial arts classes, training, sparring, and related activities at Core Combat Sports involves inherent risks, including but not limited to physical injury, bruises, sprains, fractures, concussions, and in rare cases, serious injury or death. I voluntarily choose to participate (or allow my child to participate) in these activities, assuming all risks involved. I understand that while Core Combat Sports takes all reasonable precautions to ensure a safe environment, injuries may still occur.
 

WAIVER AND LIMITATION OF LIABILITY
I, on behalf of myself (or my minor child), agree to release and hold harmless Core Combat Sports, its owners, instructors, employees, agents, and any associated entities from liability for injuries or damages arising from the inherent risks of martial arts training. I acknowledge that this waiver does not exclude or limit liability for death or personal injury caused by the negligence of Core Combat Sports, its instructors, or staff, in accordance with the Unfair Contract Terms Act 1977 and UK consumer law.
 

MEDICAL DECLARATION & CONSENT TO TREATMENT
I certify that I (or my child) am physically fit and have no medical conditions that would prevent safe participation in martial arts activities. I understand it is my responsibility to consult a physician regarding any health concerns before participating. In the event of an injury or medical emergency, I authorise Core Combat Sports staff to seek medical attention on my (or my child’s) behalf and agree to be responsible for any associated costs.
 

JUNIOR PARTICIPANT AGREEMENT (If under 18)
I, as the parent or legal guardian of the minor participant, consent to their participation in martial arts training and assume all risks and responsibilities on their behalf. I understand that my child may engage in controlled sparring and physical contact as part of their training. I acknowledge the importance of safety guidelines and agree that my child will adhere to the instructions provided by instructors. I also acknowledge that while I am consenting on their behalf, this does not affect any legal rights they may have in the future.
 

RULES AND CONDUCT
I agree to abide by all safety rules, guidelines, and instructions provided by Core Combat Sports instructors. I understand that failure to comply with these rules may result in suspension or termination of participation without a refund.
